    Summary

    A rare congenital dermoid cyst presented as a calf mass in a child after birth. Surgical removal offered a good prognosis with a low risk of recurrence.

    Background:

    • Congenital anomalies require careful monitoring and diagnosis.
    • Accessory limb structures can present unique clinical challenges.

    Observation:

    • A slowly enlarging posterior calf mass was noted in a male infant following resection of an accessory left foot at birth.
    • The mass was surgically excised at age 8 due to significant enlargement.

    Findings:

    • Histologic examination confirmed the mass as a dermoid cyst.
    • The cyst was lined with stratified squamous epithelium and contained cutaneous adnexal structures, including eccrine sweat glands.

    Implications:

    • Congenital dermoid cysts in the extremities are uncommon findings.
    • Complete surgical excision typically leads to a favorable prognosis with rare recurrence, making it a manageable condition.
