    Background:

    • Reiter's disease (RD), now often referred to as reactive arthritis, is a form of seronegative arthritis.
    • Childhood presentations of RD are less common and can be challenging to diagnose.
    • The human leukocyte antigen (HLA)-B27 antigen is strongly associated with RD.

    Observation:

    • Three Canadian Indian boys (ages 2, 14, 15) presented with arthritis, joint effusions, and tenderness.
    • The two younger boys exhibited the classic triad of RD: arthritis, conjunctivitis, and urethritis.
    • The eldest boy had dysuria and signs suggestive of keratoderma blennorrhagica.

    Findings:

    • All three boys tested positive for the HLA-B27 antigen.
    • Chlamydial infection inclusions were detected in assays from the two youngest patients.
    • Review of previously documented childhood RD cases was conducted.

    Implications:

    • Flexible diagnostic criteria may improve the identification of seronegative arthritis in children.
    • Recognizing atypical presentations of RD is crucial for timely intervention.
    • The association with HLA-B27 and potential chlamydial triggers warrants further investigation in pediatric populations.
