Summary

Patients with hip microinstability show greater range of motion (ROM) than those without. A flexion + rotation arc of 200° or more strongly indicates hip microinstability in symptomatic individuals.

Background:

  • Hip microinstability is a growing cause of pain and disability in young adults.
  • Limited understanding exists regarding passive range of motion (ROM) differences in hip microinstability.
  • Ligamentous and capsular laxity are hypothesized to cause ROM variations in hip conditions.

Purpose of the Study:

  • To investigate differences in passive hip range of motion (ROM) between patients with and without hip microinstability.
  • To determine if ROM measurements can predict hip microinstability.
  • To compare ROM in patients with isolated microinstability, femoroacetabular impingement (FAI), and controls.

Main Methods:

  • Retrospective review of 263 hips from arthroscopy patients (2012-2018).
  • Categorization into isolated instability, FAI, FAI + instability, and control groups.
  • Measurement of hip flexion, internal rotation, external rotation, and combined arcs (total rotation, flexion + rotation, flexion + 2x rotation).

Main Results:

  • Patients with hip microinstability exhibited significantly greater ROM across all measured planes compared to controls.
  • Multivariable analysis revealed significant differences in flexion and flexion + rotation arc among all groups.
  • A flexion + rotation arc ≥200° demonstrated high predictive value (89.1% PPV) for hip microinstability in symptomatic patients.

Conclusions:

  • Hip microinstability is associated with significantly increased passive range of motion.
  • A combined flexion and rotation arc of ≥200° is a strong indicator of hip microinstability in symptomatic individuals.
  • Predicting instability is challenging with a flexion + rotation arc below 200°.
