Life (Basel, Switzerland)·2026

This study reports the successful robotic partial nephrectomy (RPN) of a large renal leiomyoma, a rare kidney tumor. This minimally invasive approach preserves healthy kidney tissue, expanding options for benign renal mass management.

Area of Science:

  • Urology
  • Surgical Oncology
  • Nephrology

Background:

  • Renal leiomyomas are rare benign mesenchymal tumors originating from kidney smooth muscle cells.
  • Surgical excision of large renal tumors can be complex, especially near major blood vessels.

Observation:

  • A 59-year-old female with a massive 8 cm exophytic renal mass abutting the inferior vena cava underwent robotic partial nephrectomy (RPN).
  • The procedure involved careful tumor dissection, intraoperative ultrasound, and parenchyma reconstruction.
  • The patient had a warm ischemia time of 19 minutes and was discharged on postoperative day 2.

Findings:

  • Robotic partial nephrectomy (RPN) was successfully performed for a large renal leiomyoma (approximately 8 cm).
  • Pathology confirmed a leiomyoma of genital stromal origin with hyalinization and calcification.

  • No intraoperative complications occurred, with estimated blood loss of 250 ml.
  • Implications:

    • This case represents the first description of RPN for a large renal leiomyoma.
    • Robotic-assisted surgery broadens the application of minimally invasive nephron-sparing surgery for benign renal masses.
    • This technique is crucial for managing complex benign tumors without sacrificing healthy renal parenchyma.
