Standardizing periprocedural anticoagulation management: a stewardship initiative.

Managing anticoagulation peri-procedurally is challenging. This study implemented a standardized electronic health record (EHR) process, improving patient safety and reducing adverse events for anticoagulated patients undergoing procedures.
