    Area of Science:

    • Obstetrics and Gynecology

    Background:

    • Cervical examination in the early third trimester is a potential method for identifying pregnancies at risk for preterm delivery.
    • Previous research has explored various factors associated with preterm birth, but a clear predictive tool remains crucial.

    Purpose of the Study:

    • To evaluate the efficacy of cervical examination between 26 and 30 weeks' gestation in identifying women at risk for delivery before 34 weeks' gestation.
    • To assess the association between cervical dilation and other factors like parity and prior preterm delivery with early preterm birth.

    Main Methods:

    • A prospective study involving 185 consecutive women undergoing blinded cervical examinations between 26 and 30 weeks' gestation.
    • Data collection included cervical dilation measurements, parity, and history of preterm delivery.

    Main Results:

    • 8% of women (15 out of 185) exhibited cervical dilation of 2-3 cm.
    • Women with 2-3 cm cervical dilation had a significantly higher incidence of delivery before 34 weeks (27%) compared to those with 0-1 cm dilation (2%).
    • Parity and prior preterm delivery were associated with cervical dilation, but cervical dilation remained an independent risk factor for early delivery.

    Conclusions:

    • Early third-trimester cervical examination serves as a valuable adjunct in identifying women at increased risk for preterm delivery.
    • This method can aid in targeted interventions and improved management of high-risk pregnancies.
