Background:

  • Early breast cancer (BC) management often involves breast-conserving surgery.
  • Assessing optimal adjuvant radiotherapy techniques for low-risk BC is crucial.

Purpose of the Study:

  • To evaluate clinical response and cosmetic outcomes of full-dose intraoperative electron radiotherapy (IOERT) in early breast cancer patients undergoing lumpectomy.
  • To determine the safety and efficacy of IOERT in this specific patient cohort.

Main Methods:

  • A prospective analysis of 162 early breast cancer patients (>60 years, tumor size ≤2 cm, luminal A, node-negative) treated with lumpectomy and 21 Gy IOERT.
  • Clinical, cosmetic, and instrumental follow-ups were conducted at specified intervals.
  • Toxicity assessed using CTCAE v.4.03 and EORTC-RTOG; cosmetic outcomes graded by Harvard/NSABO/RTOG scale.

  • Locoregional relapse occurred in 2.5% of patients; no distant metastases were reported.
  • No acute radiation complications; 3.7% experienced late G2-3 toxicity, and 3.7% had poor cosmetic results.

  • The technique demonstrates a favorable balance of low local recurrence rates and good cosmetic outcomes.
  • These findings support the routine clinical application of IOERT in this patient group.
