Background:

Fibromyalgia (FM) is a chronic pain syndrome with higher prevalence in females.
Current disease severity assessments, like the Revised Fibromyalgia Impact Questionnaire (FIQR), were validated on predominantly female samples.
Potential gender bias in FM assessment tools requires investigation.

Purpose of the Study:

To compare Revised Fibromyalgia Impact Questionnaire (FIQR) item scores between male and female fibromyalgia patients.
To evaluate potential gender bias in the FIQR as a fibromyalgia severity index.

Main Methods:

A case-control study involving 78 fibromyalgia patients (39 males, 39 females) matched for age and disease duration.
Patients completed an online survey including demographic data, disease variables, and the Italian version of the FIQR.
Univariate analysis was used to compare FIQR scores between genders.

Main Results:

Female fibromyalgia patients reported significantly higher total FIQR scores and physical function domain scores.
Females scored significantly higher on 6 out of 21 FIQR items.
Specifically, females scored higher on 5 out of 9 sub-items within the physical function domain.

Conclusions:

The Revised Fibromyalgia Impact Questionnaire (FIQR) may underestimate disease impact in male fibromyalgia patients.
Further research is needed to validate FM assessment tools across genders to ensure accurate severity indexing.
