Area of Science:

  • Medical Physics
  • Radiation Oncology
  • Quality Assurance

Background:

  • Electronic Portal Imaging Devices (EPIDs) are increasingly used for patient-specific quality assurance (PSQA) and transit dosimetry.
  • Current guidelines for EPID utilization in these dosimetry applications are lacking.
  • The American Association of Physicists in Medicine (AAPM) Task Group 307 (TG-307) addresses this gap.

Purpose of the Study:

  • To comprehensively review the physics, modeling, algorithms, and clinical experience of EPID-based pre-treatment and transit dosimetry.
  • To provide specific guidelines on the uses, limitations, and correct utilization of EPIDs for dosimetry.
  • To offer recommendations for commissioning, calibration, validation, routine QA, and risk-based analysis.

Main Methods:

  • Review of currently available EPID systems and EPID-based PSQA techniques.
  • Detailed discussion of physics, modeling, and algorithms for pre-treatment and transit dosimetry.
  • Analysis of clinical experience, commissioning, calibration, validation, tolerance levels, and risk-based analysis.

Main Results:

  • Description of clinical experience, commissioning methods, and tolerances for EPID-based PSQA in pre-treatment and transit dosimetry.
  • Presentation of sensitivity, specificity, and clinical results, including error detection examples.
  • Discussion of limitations, challenges, acceptance/rejection criteria, and causes of dosimetry failures.

Conclusions:

  • TG-307 offers guidance for medical physicists on implementing commercially available EPID-based dosimetric tools.
  • Focus on patient-specific pre-treatment and transit dosimetry QA solutions.
  • Guidance is applicable to intensity modulated radiation therapy (IMRT) and volumetric modulated arc therapy (VMAT) treatments.
