    Background:

    • Migraine is a common neurological disorder.
    • Complicated migraine can present with diverse neurological symptoms.
    • Distinguishing primary migraine from secondary causes is clinically important.

    Observation:

    • 137 patients with classical or complicated migraine were studied.
    • 13 patients (9.5%) exhibited pathological alterations.
    • Pathologies included tumors, arteriovenous malformations, aneurysms, and Moya-Moya syndrome.

    Findings:

    • Migraine accompagnée was diagnosed in 11 patients, with 2 experiencing epileptic seizures.
    • One patient had ophthalmoplegic migraine, and another presented with subarachnoid hemorrhage mimicking migraine.
    • Successful surgical intervention improved outcomes for 8 patients.

    Implications:

    • Neurologists should consider extensive investigations, including cerebral angiography, for specific patient criteria.
    • These criteria include unilateral, persistent headaches, changing headache patterns, neurological deficits, seizures, and abnormal EEG/CAT findings.
    • A delayed diagnosis, up to 25 years, highlights the need for heightened clinical suspicion in complex migraine cases.
