    Background:

    • Meckel's diverticulum is a congenital anomaly, a remnant of the omphalomesenteric duct.
    • Neoplasia within Meckel's diverticulum is uncommon.
    • Malignant melanoma typically arises from melanocytes in the skin or eyes.

    Observation:

    • A middle-aged male patient presented with a rare occurrence of malignant melanoma.
    • The melanoma was located within the Meckel's diverticulum.
    • The patient had no concurrent skin or ocular lesions, making the diagnosis unusual.

    Findings:

    • This is the first reported case of malignant melanoma originating in Meckel's diverticulum.
    • Histological and immunoperoxidase analyses of other Meckel's diverticuli were performed.
    • Investigation for neural crest elements was conducted to understand the lesion's origin.

    Implications:

    • This case expands the known spectrum of neoplasms affecting Meckel's diverticulum.
    • It highlights the importance of considering rare origins for malignant melanoma.
    • Further research may elucidate the potential role of neural crest elements in such gastrointestinal tumors.
