Summary

A large cardiac lipoma in the right atrium prolapsed through the tricuspid valve. Emergency surgery successfully removed the tumor and repaired the tricuspid valve, confirming a benign cardiac lipoma diagnosis.

Background:

Atrial flutter is a common arrhythmia.
Cardiac tumors are rare but can cause significant complications.
Large right atrial masses pose a risk of valvular obstruction.

Observation:

A 56-year-old male presented with a large (8 cm) right atrial mass.
The mass prolapsed through the tricuspid valve into the right ventricle.
The patient had a history of atrial flutter.

Findings:

Emergency surgical excision of the right atrial mass was performed.
Tricuspid annuloplasty was conducted during the surgery.
Pathological examination confirmed the mass as a cardiac lipoma.

Implications:

Surgical intervention is effective for symptomatic cardiac lipomas.
Prompt diagnosis and treatment can prevent severe hemodynamic compromise.
Cardiac lipomas, though rare, should be considered in the differential diagnosis of atrial masses.
