    Summary

Multiple sclerosis (MS) can rarely present as isolated oculomotor nerve palsy. This case highlights MS as a potential diagnosis for sudden onset of third nerve paralysis with pupillary involvement.

    Area of Science:

    • Neuroscience
    • Ophthalmology
    • Neurology

    Background:

    • Multiple sclerosis (MS) is a chronic inflammatory demyelinating disease of the central nervous system.
    • Oculomotor nerve palsy, affecting eye movement and pupil size, is a known but uncommon neurological manifestation.
    • Isolated cranial nerve palsies can present diagnostic challenges, necessitating thorough investigation.

    Observation:

    • A young woman presented with acute, isolated oculomotor nerve palsy, including pupillary dilation.
    • Initial investigations and patient history did not reveal an apparent cause for the palsy.
    • The patient's subsequent clinical course led to a diagnosis of multiple sclerosis.

    Findings:

    • This case represents the first documented instance of multiple sclerosis presenting solely as an isolated oculomotor nerve paralysis.

    Related Experiment Videos

  • The findings underscore the diverse and sometimes atypical neurological presentations of MS.
  • Oculomotor nerve involvement in MS, while documented, typically occurs in conjunction with other neurological deficits.
  • Implications:

    • This case expands the spectrum of initial presentations for multiple sclerosis.
    • Clinicians should consider MS in the differential diagnosis of unexplained isolated oculomotor nerve palsies, especially in young adults.
    • Early recognition of atypical MS presentations is crucial for timely treatment and management to prevent further neurological damage.
