    Summary

Ulcerative colitis patients with extensive or long-lasting disease face a higher risk of colorectal cancer. Surveillance colonoscopy is recommended after 8-10 years for extensive colitis and 15 years for left-sided colitis.

    Area of Science:

    • Gastroenterology
    • Oncology
    • Epidemiology

    Background:

    • Ulcerative colitis (UC) is an inflammatory bowel disease associated with an increased risk of colorectal cancer (CRC).
    • Long-standing and extensive colitis are known risk factors for CRC development.
    • Understanding cancer risk in UC patients is crucial for effective surveillance strategies.

    Purpose of the Study:

    • To determine the incidence and risk factors for colorectal cancer in a large cohort of ulcerative colitis patients.
    • To evaluate the risk of extracolonic malignancies in patients with ulcerative colitis.
    • To provide evidence-based recommendations for cancer surveillance in UC.

    Main Methods:

    • A retrospective review of 1248 ulcerative colitis cases followed up to 1984.

    Related Experiment Videos

  • Analysis of colorectal cancer incidence, characteristics, and survival rates.
  • Comparison of cancer risk between extensive and left-sided colitis.
  • Assessment of extracolonic malignancy incidence.
  • Main Results:

    • Colorectal cancer was diagnosed in 6.5% of patients; extracolonic malignancy in 3.8%.
    • Higher CRC risk was observed in patients with extensive colitis (25.3% at 30 years) compared to left-sided colitis (3.7% at 30 years).
    • CRC prognosis in UC patients was similar to the general population, with a 5-year survival rate of 54%.

    Conclusions:

    • Surveillance colonoscopy should be initiated after 8-10 years of extensive colitis and 15 years of left-sided colitis.
    • Specific extracolonic malignancies, including bile duct carcinoma and leukemia, were more frequent than expected in UC patients.
    • The study reinforces the importance of regular cancer screening for ulcerative colitis patients, tailored to disease extent and duration.
