    Summary

    Posterior mediastinal lymph node enlargement from sarcoidosis is rare. This condition, without other typical signs, required surgical diagnosis in two patients.

    Background:

    • Sarcoidosis is a multisystem granulomatous disease.
    • Mediastinal lymphadenopathy is a common manifestation of sarcoidosis.

    Observation:

    • Two cases of posterior mediastinal lymph node enlargement due to sarcoidosis are presented.
    • Typical sarcoidosis indicators such as bilateral hilar enlargement, lung parenchymal disease, or extrathoracic involvement were absent in both cases.

    Findings:

    • The absence of characteristic sarcoidosis findings complicated diagnosis.
    • Diagnostic thoracotomy was necessary to confirm sarcoidosis in both patients.

    Implications:

    • This presentation highlights a rare manifestation of sarcoidosis.
    • Diagnostic challenges in atypical sarcoidosis cases may necessitate invasive procedures.
