Journal of Neurology, Neurosurgery, and Psychiatry
    |July 1, 1986

    Summary

Sarcoidosis can cause optic neuropathy, presenting as chronic progressive vision loss resistant to steroids or acute vision loss responding to steroids. Steroid dependence was common in the acute cases.

    Area of Science:

    • Ophthalmology
    • Neurology
    • Immunology

    Background:

    • Sarcoidosis is a systemic inflammatory disease that can affect the optic nerve.
    • Optic neuropathy is a potential complication of sarcoidosis, leading to vision impairment.

    Purpose of the Study:

    • To describe the clinical characteristics and treatment outcomes of patients with isolated optic neuropathy secondary to sarcoidosis.
    • To differentiate between distinct clinical presentations and responses to therapy.

    Main Methods:

    • Case series review of five patients with isolated optic neuropathy and confirmed sarcoidosis.
    • Clinical assessment of visual function, optic nerve appearance, and response to corticosteroid therapy.

    Main Results:

    • Two distinct clinical patterns were observed: chronic progressive visual loss with optic nerve thickening unresponsive to steroids, and acute/subacute visual loss responsive to steroids.
    • All patients with acute/subacute optic neuropathy developed steroid dependence.
    • Clinical presentation did not mimic optic neuritis associated with multiple sclerosis.

    Conclusions:

    • Isolated optic neuropathy in sarcoidosis exhibits variable clinical courses and treatment responses.
    • Prompt steroid treatment is effective for acute presentations, but often leads to steroid dependence.
    • Distinguishing sarcoid optic neuropathy from other causes, like multiple sclerosis, is crucial for appropriate management.
