    Background:

    • Pontine tegmental hemorrhage is a rare type of stroke affecting the brainstem.

    • A 47-year-old hypertensive male presented with acute headache, cranial nerve palsies, weakness, and dysmetria.
    • Despite these focal neurological signs, the patient's alertness and overall mental status were completely preserved.

    Findings:

    • Computerized tomography (CT) confirmed a bilateral pontine tegmental hemorrhage.
    • This represents the first documented case of pontine tegmental hemorrhage with preserved consciousness.

    Implications:

    • This case challenges typical presentations of brainstem strokes, suggesting preserved alertness is possible.
    • Further research into the neuroanatomical correlates of consciousness in brainstem lesions is warranted.
    • Highlights the importance of detailed neurological examination even in patients with seemingly preserved mental status.
