Body composition in quadriplegic patients.

    Summary

Many quadriplegic patients exhibit malnutrition, indicated by reduced body cell mass (BCM) relative to extracellular mass (ECM). This nutritional state correlates with spinal cord injury level, not duration.

    Area of Science:

    • Nutrition
    • Physiology
    • Rehabilitation Medicine

    Background:

    • Quadriplegia, resulting from spinal cord injury, often leads to extensive skeletal muscle atrophy.
    • Assessing the nutritional status of individuals with quadriplegia is crucial for effective management and rehabilitation.

    Purpose of the Study:

    • To evaluate the body composition and nutritional state of patients with quadriplegia.
    • To compare the body composition of quadriplegic patients with that of healthy volunteers.

    Main Methods:

    • Utilized a multiple isotope dilution technique to determine body composition in 12 quadriplegic patients and 25 healthy volunteers.
    • Measured body cell mass (BCM) and extracellular mass (ECM) to assess nutritional status.

    Main Results:

    • A significant percentage (58%) of quadriplegic patients displayed body composition characteristic of malnutrition, with reduced BCM relative to ECM.
    • Malnourished quadriplegic patients had significantly different body composition compared to healthy controls and well-nourished quadriplegic patients.
    • Nutritional status correlated significantly with the level of spinal cord injury, but not with the duration of quadriplegia.

    Conclusions:

    • A substantial proportion of quadriplegic patients experience malnutrition, contrary to expectations of generally reduced body size.
    • The level of spinal cord injury is a key factor influencing the nutritional state in quadriplegia.
    • These findings highlight the importance of nutritional assessment in the management of spinal cord injury patients.
