Background:

  • Coumel tachycardia is an uncommon supraventricular tachycardia (SVT) primarily affecting infants and children.
  • It is characterized by an accessory pathway with slow retrograde conduction, leading to a distinct ECG pattern (long RP' interval, negative P waves).

Observation:

  • A retrospective review of five pediatric patients (mean age 11 years) with Coumel tachycardia was conducted.
  • Patients experienced persistent tachycardia refractory to antiarrhythmic drugs, with one developing cardiomyopathy.
  • Incessant tachycardia with >85% density was noted on 24-hour Holter monitoring.

Findings:

  • All five pediatric patients underwent successful radiofrequency catheter ablation with no complications.
  • A mean of 5 applications were needed in a single session.
  • Following a mean 24-month follow-up, all patients remained asymptomatic and free from recurrence without medication.

Implications:

  • Coumel tachycardia poses a significant risk of cardiomyopathy due to its persistent nature and resistance to medical therapy.
  • Radiofrequency catheter ablation is a highly effective and safe treatment option for pediatric Coumel tachycardia.
  • Prompt consideration of catheter ablation is recommended for affected children based on individual assessment.
