Summary

Transumbilical breast augmentation (TUBA) using prefilled silicone implants is feasible with various implant types and dissection planes. Full endoscopy TUSBA reduces complication rates compared to endoscope-assisted methods.

Area of Science:

  • Plastic Surgery
  • Minimally Invasive Surgery
  • Aesthetic Surgery

Background:

  • Transumbilical breast augmentation (TUBA) faces technical challenges with diverse silicone implant options and dissection planes.
  • Prefilled silicone implants offer potential but require refined surgical techniques for optimal outcomes.

Purpose of the Study:

  • To demonstrate the feasibility of TUBA using various prefilled silicone implants (TUSBA).
  • To evaluate TUSBA applicability across subglandular, subfascial, and dual-plane implantation.
  • To compare outcomes between endoscope-assisted and full-endoscopy TUSBA techniques.

Main Methods:

  • Retrospective review of 74 patients undergoing TUSBA between June 2016 and April 2021.
  • Utilized smooth, textured, or nanotextured round or anatomic silicone implants up to 500 mL.
  • Compared endoscope-assisted blunt dissection with full-endoscopy dissection for pocket creation and bleeding control.

Main Results:

  • Aesthetically pleasing outcomes were achieved in both endoscope-assisted and full-endoscopy groups.
  • The endoscope-assisted group had a 4.6% revision rate, while the full-endoscopy group required no revisions.
  • No excessive postoperative pain was reported in either group.

Conclusions:

  • Transumbilical breast augmentation with prefilled silicone implants is feasible and adaptable to multiple dissection planes.
  • The full-endoscopy technique shows promise in reducing complication rates associated with TUSBA.
  • TUSBA accommodates a wide range of silicone implant types, shapes, and sizes.
