    Background:

    • Infectious mononucleosis, typically caused by Epstein-Barr virus (EBV), is a common viral illness.
    • Hepatitis is a known complication of infectious mononucleosis.

    Observation:

    • A 22-year-old male presented with symptoms consistent with hepatitis.
    • The patient was diagnosed with infectious mononucleosis.
    • Despite extensive evaluation, including liver biopsy, no alternative cause for the patient's exudative ascites was identified.

    Findings:

    • The patient's exudative ascites and hepatitis resolved spontaneously as the acute infectious mononucleosis infection subsided.
    • No chronic liver disease was evident 12 months post-illness.
