    Background:

    • Sarcoidosis is a multisystem inflammatory disease with variable clinical presentations.
    • Chronic sarcoidosis, defined as disease lasting over five years, can lead to persistent disability.
    • Ophthalmic involvement is a significant complication of sarcoidosis, impacting patient morbidity.

    Purpose of the Study:

    • To investigate the prevalence and clinical course of ophthalmic involvement in patients with chronic sarcoidosis.
    • To determine the relationship between ocular disease manifestations and the systemic disease course.
    • To identify prognostic factors for visual outcomes in sarcoidosis-related eye disease.

    Main Methods:

    • Retrospective analysis of 183 patients diagnosed with chronic sarcoidosis (≥5 years duration).
    • Detailed review of ophthalmic examinations and clinical histories.
    • Longitudinal follow-up of systemic and ocular disease progression for a mean of 13 years.

    • Ophthalmic involvement was observed in 47 patients (26%), with uveitis developing in 35 (19%).

    • Early detection and management of chronic uveitis and secondary glaucoma are crucial to prevent severe visual impairment in sarcoidosis patients.
