Purpose of the Study:

  • To determine if CUSUM charts of operative time complement the Accreditation Council for Graduate Medical Education (ACGME) Milestones evaluations.
  • To aid residency programs in assessing the practice readiness of graduating general surgery residents.

Main Methods:

  • ACGME Milestone evaluations for performance of operations and procedures (POP) and objective metrics (operative time, case type, complexity) were extracted for 15 residents (PGY4-5) across three procedures.
  • CUSUM charts were computed for each resident and procedure, defining a learning plateau as at least 4 cases with minimal deviation near the target performance.
  • Data included 2,446 cases: cholecystectomy (N=1234), colectomy (N=507), and inguinal hernia (N=705).

Main Results:

  • All residents met overall ACGME POP graduation targets by the end of their chief year.
  • Analysis of CUSUM charts revealed varied development processes among residents, with only 46.7% reaching a learning plateau in all three procedures by graduation.
  • Three CUSUM chart patterns were observed: skewed, bimodal, and peaks and valleys distributions.

Conclusions:

  • CUSUM charts of operative time serve as a valuable complement to ACGME Milestones evaluations for assessing surgical residents.
  • Integrating both assessment methods allows for a more holistic evaluation of graduating residents' practice readiness.
  • This combined approach can inform residency programs' recommendations for residents transitioning to independent practice.
