Summary

Bilateral chronic otitis media presents with more frequent concomitant conditions and larger cholesteatomas than unilateral disease. However, surgical outcomes are similar for both conditions, though operative timing and unique challenges for single-hearing ears require consideration.

Background:

Chronic otitis media (COM) affects numerous individuals globally.
Bilateral COM presents unique clinical and surgical considerations compared to unilateral COM.

Purpose of the Study:

To compare clinical presentations and surgical outcomes of bilateral COM versus unilateral COM.
To identify associated conditions and cholesteatoma characteristics in bilateral COM.

Main Methods:

Retrospective comparison of 86 patients with bilateral COM and 641 patients with unilateral COM.
Analysis of clinical findings, concomitant conditions, cholesteatoma size, and surgical results.

Main Results:

Bilateral COM cases showed significantly higher prevalence of cleft palate, rhinitis, and adenotonsillitis.
Cholesteatomas were larger in bilateral COM cases.
Overall surgical success rates were comparable between bilateral and unilateral COM groups.

Conclusions:

Bilateral COM is associated with specific comorbidities and more extensive disease.
Surgical management yields similar results for both unilateral and bilateral COM.
Special attention is needed for surgical timing and managing the single-hearing ear in bilateral COM.
