Summary

Transrectal ultrasound guided prostate needle biopsy may cause temporary erectile dysfunction, especially with repeat procedures. However, long-term erectile function is not significantly impacted by prostate needle biopsies in aging men.

Background:

  • The relationship between transrectal ultrasound guided prostate needle biopsy and erectile dysfunction (ED) is not fully understood.
  • This study investigates if prostate needle biopsies contribute to the development or worsening of ED.

Purpose of the Study:

  • To assess the short-term and long-term impact of transrectal ultrasound guided prostate needle biopsy on erectile function.
  • To evaluate the association between multiple prostate biopsies and erectile dysfunction using the IIEF-5 score.

Main Methods:

  • 826 men undergoing 10 to 12-core prostate needle biopsy were assessed using the IIEF-5 questionnaire.
  • Erectile dysfunction was categorized (any, mild, moderate, severe).
  • The impact of repeat biopsies on ED was analyzed.

Main Results:

  • 29% of men had undergone one or more biopsies, 20% had two or more.
  • Age and prior biopsies were associated with ED on univariate analysis.
  • Repeat biopsy within 12 months correlated with worse ED; however, long-term function was not significantly predicted by biopsy history.

Conclusions:

  • Prostate needle biopsy may predict transient ED (less than 1 year) in the short term.
  • Prostate needle biopsy does not predict long-term erectile dysfunction in aging men.
