    Summary

    Neodymium Yttrium Aluminum Garnet (Nd:YAG) laser surgery offers a viable conservative approach for malignancies in solitary kidneys. This renal parenchymal-sparing technique showed no recurrence in patients with transitional cell or renal cell carcinoma.

    Background:

    • Conservative management of renal malignancies in solitary kidneys presents challenges.
    • Traditional surgical options may be limited in elderly patients or those with compromised renal vasculature.

    Observation:

    • Six patients with solitary kidney malignancies underwent surgery using the Neodymium Yttrium Aluminum Garnet (Nd:YAG) laser.
    • Three patients with transitional cell carcinoma were treated with Nd:YAG laser via nephroscope before partial nephrectomy.
    • Three patients with renal cell carcinoma had tumors excised using Nd:YAG laser combined with standard surgical techniques.

    Findings:

    • No evidence of recurrent transitional cell carcinoma was observed in three patients during follow-up periods of 28, 18, and 6 months.
    • No evidence of recurrent renal cell carcinoma was detected on CT scans in three elderly patients with compromised renal vasculature at 16, 14, and 3 months post-surgery.
    • The Nd:YAG laser facilitated parenchymal-sparing surgery in cases where bench surgery or renal artery occlusion was not feasible.

    Implications:

    • The Nd:YAG laser provides a minimally invasive, effective option for treating renal tumors in solitary kidneys.
    • This approach preserves renal function in patients with limited surgical alternatives.
    • Further research can explore the long-term efficacy and broader application of laser-assisted renal surgery.
