    Background:

    • Thymic seminomas typically resemble testicular seminomas histologically.
    • Secondary changes can complicate the diagnosis of thymic seminomas.

    Observation:

    • Three cases of thymic seminoma exhibited significant secondary changes including reactive follicular hyperplasia, large cysts, epithelioid granulomas, and fibrosis.
    • These changes made the seminoma component indistinct in some instances.

    Findings:

    • Immunoperoxidase staining revealed thymic epithelial cells associated with non-staining seminoma cells.
    • Seminoma cells were reactive with placental alkaline phosphatase.
    • This combination of secondary changes was not observed to a similar degree in other thymic or mediastinal tumors.

    Implications:

    • Accurate diagnosis of thymic seminoma variants is essential.
    • Distinguishing these variants from other mediastinal tumors is critical due to the favorable prognosis of thymic seminomas.
