Left atrial plication (LAP) significantly reduces blood flow stagnation in enlarged left atria (LA) compared to left atrial appendage resection (LAAR). This surgical technique offers a promising solution for improving blood flow in patients with enlarged LA and chronic atrial fibrillation.
Area of Science:
- Cardiology
- Medical Imaging
- Computational Fluid Dynamics
Background:
- Enlarged left atrium (LA) is associated with blood flow stagnation, increasing risks in patients with chronic atrial fibrillation.
- Left atrial appendage resection (LAAR) is a common intervention, but its efficacy in reducing stagnation is debated.
- Left atrial plication (LAP) is a newer technique for managing enlarged LA.
Purpose of the Study:
- To evaluate blood flow stagnation in enlarged left atria (LA).
- To demonstrate the effectiveness of left atrial plication (LAP) in alleviating blood flow stagnation.
- To compare the efficacy of LAP with virtual left atrial appendage resection (LAAR).
Main Methods:
- Computational fluid dynamics (CFD) analysis was performed on five patients with enlarged LA undergoing mitral valve surgery and LAP.
- Preoperative and postoperative imaging (CT and 4D-flow MRI) were used for CFD modeling.
- CFD analysis assessed blood flow stagnation in three scenarios: preoperative, virtual LAAR, and LAP.
Main Results:
- Preoperatively, significant blood flow stagnation was observed in the enlarged LA and left atrial appendage.
- Following LAP, average stagnation volume decreased to 35.88 ± 8.20, and constant stagnation volume reduced to 21.14 ± 7.70.
- LAP demonstrated significantly greater reduction in both average and constant stagnation volumes compared to virtual LAAR (p < 0.05).
Conclusions:
- Blood flow stagnation is a significant issue in enlarged left atria (LA), particularly in the left atrial appendage.
- Left atrial plication (LAP) effectively improves blood flow dynamics within the enlarged LA.
- CFD analysis confirms LAP as a superior method for reducing blood flow stagnation compared to LAAR alone.
