Background:

  • Social cognition (SC) is crucial for social interaction but is not well-defined in early multiple sclerosis (MS).
  • It remains unclear if SC deficits in MS are independent of general cognitive impairment.

Purpose of the Study:

  • To compare social cognition (SC) measures in early relapsing-remitting MS (RRMS) patients versus a matched control group.
  • To investigate the relationship between SC, cognitive function, and clinical factors in early MS.

Main Methods:

  • Cross-sectional study comparing 38 early RRMS patients with 38 healthy controls.
  • Assessed SC using Emotion Recognition Task, Reading the Mind in the Eyes Test (RMET), and Faux Pas Test (FPT).
  • Evaluated general cognition, depression, anxiety, fatigue, and quality of life.

Main Results:

  • 34.2% of early RRMS patients exhibited altered social cognition (SC).
  • MS patients showed poorer performance in cognitive and affective theory of mind (ToM) compared to controls; no differences in facial emotion recognition (FER).
  • Cognitive ToM and FER correlated with cognitive functions, but affective ToM did not; poor quality of life was linked to altered SC.

Conclusions:

  • Social cognition impairment, particularly ToM deficits, is prevalent in early RRMS.
  • Cognitive ToM and FER correlate with general cognitive status, while affective ToM appears independent of other cognitive functions in early MS.
