    Background:

    Purpose of the Study:

    • To evaluate the long-term impact of cisplatin on renal function in patients with testicular cancer.
    • To determine if cisplatin causes permanent damage to glomerular filtration rate (GFR) and renal plasma flow.
    • To identify potential long-term tubular defects associated with cisplatin treatment.

    Main Methods:

    • Follow-up study of 22 patients 16-52 months after cisplatin chemotherapy for disseminated nonseminomatous testicular cancer.
    • Median cumulative cisplatin dose was 452 mg/m2.
    • Renal function assessed by glomerular filtration rate (51Cr-EDTA clearance) and effective renal plasma flow (125I-hippuran clearance); serum creatinine, serum magnesium, and urinary beta-2-microglobulin levels were also measured.

    Main Results:

    • A median 12.5% reduction in glomerular filtration rate (GFR) was observed compared to pre-treatment levels (P < 0.01).
    • Effective renal plasma flow was also reduced.
    • Serum creatinine levels increased during treatment but stabilized afterward; serum magnesium and beta-2-microglobulin remained normal.

    Conclusions:

    • Cisplatin treatment leads to a moderate and permanent reduction in GFR.
    • No significant long-term tubular defects were detected in patients treated with cisplatin.
    • Renal function stabilizes post-treatment, despite a persistent decrease in GFR.
