Mesial temporal lobe epilepsy (MTLE) patients with gliosis showed poorer surgical outcomes and distinct insular hypometabolism. Prioritizing hypometabolic regions in [18F]FDG PET scans aids presurgical evaluation for better epilepsy surgery predictions.
