Background:

Biliary duct strictures can indicate malignancy, particularly in patients with a history of metastatic pancreatic cancer.
Colloid carcinoma (CC) of the pancreas is rare, with limited published data, and no prior reports of biliary tract recurrence.
Pancreatic cancer surveillance protocols require careful consideration for potential metastatic or recurrent disease.

Observation:

A patient with a history of pancreatic cancer developed a new biliary duct stricture.
The stricture was diagnosed as intrahepatic biliary colloid carcinoma.
This recurrence was noted two years after the patient received their last dose of immunotherapy for pancreatic CC.

Findings:

This case represents a unique presentation of recurrent biliary colloid carcinoma.
The findings suggest that the biliary tract is a potential site for late recurrence of pancreatic CC.
The diagnosis was confirmed through histopathological examination.

Implications:

This case underscores the importance of considering rare recurrence patterns in cancer surveillance.
It prompts a re-evaluation of optimal surveillance strategies for patients with a history of pancreatic cancer, especially CC.
Further research may be needed to understand the behavior and management of biliary tract recurrences of pancreatic CC.
