Outcomes included recurrent choledocholithiasis, need for mechanical lithotripsy, and adverse events over 36 months.

Main Results:

The overall incidence of recurrent choledocholithiasis was 13.6% for ECPP and 22.1% for limited EST plus EPLBD (P=0.204).
The ECPP group with successful SO function restoration (ECPP-A) showed a significantly lower incidence of recurrent choledocholithiasis (5.1% vs. 22.1%, P=0.020).
ECPP demonstrated successful restoration of SO function in the ECPP-A subgroup.

Conclusions:

ECPP is a safe and effective treatment for CBDS.
ECPP, particularly when restoring SO function, significantly reduces the incidence of recurrent choledocholithiasis compared to limited EST plus EPLBD.
