    Background:

    • False aneurysms are a rare but serious complication following carotid endarterectomy.
    • Infection is a significant precipitating factor in the development of these false aneurysms.

    Observation:

    • Two cases of post-carotid endarterectomy false aneurysms are presented, both linked to infection.
    • One patient required common carotid artery ligation due to persistent infection and recurrent false aneurysm.

    Findings:

    • The overall incidence of false aneurysms post-carotid endarterectomy is 0.30%.
    • Infection was identified as the primary cause in 50% of cases with primary closure without patch grafting.
    • Carotid ligation significantly increased neurological deficit risk (50% morbidity/mortality).

    Implications:

    • Saphenous vein patch grafting, synthetic sutures, and aggressive antibiotic use are recommended for managing false aneurysms.
    • Avoiding carotid ligation is crucial to minimize neurological complications.
