The auditory ossicles of the middle ear transmit sounds from the air as vibrations to the fluid-filled cochlea. The auditory ossicles consist of two malleus (hammer) bones, two incus (anvil) bones, and two stapes (stirrups), one on each side. These bones develop during the fetal stage and are the ones to ossify first. They are fully mature at birth and do not grow afterward.

Background:

Purpose of the Study:

  • To evaluate the hearing outcomes of incudo-stapedial joint reconstruction using conchal cartilage grafts.
  • To assess the efficacy of autologous conchal cartilage in tympanoplasty for chronic otitis media.

Main Methods:

  • Fifty-three patients with chronic otitis media and incudostapedial necrosis underwent tympanoplasty with conchal cartilage interposition.
  • Pre-operative and 12-week post-operative Pure Tone Audiograms were used to analyze hearing outcomes.

Main Results:

  • A statistically significant improvement of 14.66 dB in air conduction was observed (p=0.0001).
  • Bone conduction showed statistically insignificant improvement.
  • 98.1% of patients achieved post-operative air conduction thresholds ≤40 dB.

Conclusions:

  • Autologous conchal cartilage is an effective, biocompatible, and cost-effective graft for incudo-stapedial joint reconstruction.
  • This technique offers significant hearing improvement in patients with chronic otitis media.
  • Conchal cartilage provides hearing results comparable to other graft materials.
