    Summary

    Ceftriaxone treatment was ineffective for chlamydial urethritis in men. Chlamydia trachomatis infections can be asymptomatic for weeks, with pyuria better indicating infection than Gram stain.

    Background:

    • Chlamydia trachomatis is a common cause of urethritis.
    • Asymptomatic infections can pose diagnostic and treatment challenges.

    Purpose of the Study:

    • To evaluate the efficacy of ceftriaxone for treating asymptomatic Chlamydia trachomatis urethritis in men.
    • To assess the duration of asymptomatic infection and diagnostic markers.

    Main Methods:

    • Ten men with asymptomatic C. trachomatis urethritis were treated with 1g intramuscular ceftriaxone.
    • Follow-up included clinical assessment and culture at least 21 days post-therapy.
    • Urine pyuria and urethral Gram stain leukocyte counts were analyzed.

    Main Results:

    • Seven of eight men remained culture-positive post-treatment.
    • One patient developed symptomatic non-gonococcal urethritis 18 days after initial diagnosis.
    • Pyuria was present in 17/18 visits, while Gram stain leukocyte counts were elevated in 10/29 visits.

    Conclusions:

    • Intramuscular ceftriaxone (1g) is ineffective for treating male chlamydial urethritis.
    • C. trachomatis urethral infections can remain asymptomatic for 21-45 days.
    • Urinalysis-detected pyuria is a more reliable indicator of infection than urethral Gram stain in this population.
