    Background:

    • Intracranial meningiomas are common primary brain tumors.
    • Cystic variations of meningiomas are rare and pose diagnostic difficulties.
    • Pre-computed tomography era diagnosis of cystic meningiomas was often incidental.

    Purpose of the Study:

    • To analyze unusual cases of intracranial meningioma with associated cysts.
    • To evaluate the diagnostic accuracy of imaging modalities for cystic meningiomas.
    • To discuss pathogenetic mechanisms and diagnostic strategies for cystic meningiomas.

    Main Methods:

    • Retrospective review of 18 cases of intracranial meningioma with cysts.
    • Analysis of pre-operative imaging including computed tomography (CT) and selective angiography.
    • Correlation of radiologic findings with pathological diagnoses.

    Main Results:

    • Computed tomography identified cysts in 14 of 15 patients studied.
    • Peritumoral cysts were seen in 8 patients, intratumoral cysts in 6, and one totally cystic meningioma.
    • Preoperative diagnosis was correct in only 4 cases using CT; angiography improved diagnosis in 6 additional cases.

    Conclusions:

    • Cyst formation in meningiomas involves complex pathogenetic mechanisms.
    • Radiologic patterns of cystic meningiomas vary depending on cyst location and type.
    • Accurate preoperative assessment is essential to avoid misdiagnosis of cystic intracranial meningiomas.
