    Summary

    This study followed 67 seronegative arthritis patients, finding that many cases mimic seropositive rheumatoid arthritis. Synovial tissue analysis revealed distinct morphological patterns correlating with disease progression in seronegative arthritis.

    Background:

    • Seronegative arthritis presents diagnostic challenges, often mimicking other rheumatic conditions.
    • Understanding the long-term course and pathological features of seronegative arthritis is crucial for effective management.

    Observation:

    • A 7.6-year follow-up of 67 seronegative arthritis patients was conducted.
    • Four patients received revised diagnoses including ankylosing spondylitis and Reiter's syndrome.
    • Sixteen patients (24%) exhibited seropositive rheumatoid arthritis features, while 47 remained seronegative.

    Findings:

    • Among seronegative patients, 30 showed disease courses similar to seropositive rheumatoid arthritis.
    • Acute onset with polyarticular involvement and high inflammatory/immunological activity predicted disease progression.
    • Synovial biopsy in 20 patients did not reveal classical rheumatoid synovitis, even in chronic cases.

    Implications:

    • Two distinct morphological variants of synovial tissue changes were identified in seronegative RA, correlating with benign and progressive disease courses.
    • These findings suggest unique pathological pathways in seronegative arthritis, distinct from classical rheumatoid arthritis.
    • Further research into these morphological differences may lead to targeted therapies for seronegative arthritis subtypes.
