Summary

Hypothermic machine perfusion (HMP) shows promise for preserving uterus grafts in donation after brainstem death (DBD) transplantation. This technique may reduce uterus graft dysfunction and increase donor organ availability for uterus transplantation (UT).

Area of Science:

  • Reproductive Medicine
  • Transplantation Surgery
  • Organ Preservation

Background:

  • Uterus transplantation (UT) offers a solution for absolute uterine infertility.
  • Donation after brainstem death (DBD) poses preservation challenges due to prolonged ischemia.
  • Static cold storage is the current standard for uterus preservation.

Discussion:

  • Hypothermic machine perfusion (HMP) was investigated as an alternative preservation method for uterus grafts.
  • The study utilized a porcine model to assess HMP feasibility in a DBD context.
  • HMP involves perfusing the uterus with a preservation solution for an extended period.

Key Insights:

  • HMP demonstrated successful preservation of a porcine uterus graft for 18 hours.
  • The transplanted uterus showed viable macroscopic appearance and comparable vascularization to the native uterus.
  • This technique could potentially mitigate uterus graft dysfunction and improve outcomes.

Outlook:

  • HMP may extend cold ischemia times without compromising uterine graft viability.
  • Further validation in human clinical trials is necessary.
  • This approach could expand the donor pool for uterus transplantation.
