Acute Pyelonephritis I: Introduction01:27

Acute Pyelonephritis I: Introduction

21
Pyelonephritis is a bacterial infection that primarily affects the renal parenchyma and collecting system, including the renal pelvis, tubules, and interstitial tissue of one or both kidneys. It can be classified as either acute—a sudden, severe infection—or chronic, which refers to long-term or recurrent kidney infections. The primary cause of acute pyelonephritis (APN) is bacterial infection, with Escherichia coli accounting for approximately 70-80% of cases. Other bacteria, such...

Acute Pyelonephritis II: Diagnostic Studies and Management

16
Introduction:For diagnosing acute pyelonephritis, a comprehensive patient history is collected to identify symptoms such as dysuria, frequent or urgent urination, flank pain, or costovertebral angle (CVA) tenderness that may suggest a kidney infection.Physical ExaminationDuring the physical examination, CVA tenderness is assessed. This involves gentle percussion over the costovertebral angle, where tenderness often indicates a kidney infection.Diagnostic TestsUrinalysis: Used to identify white...

Pulmonary Tuberculosis III01:31

Pulmonary Tuberculosis III

363
Tuberculosis (TB) is a contagious infection primarily affecting the lung parenchyma but which can also affect other body parts. TB can be classified based on disease development, presentation, and the affected anatomical site.
The first classification is based on the development of the disease, and it includes the following categories:

Area of Science:

  • Nephrology
  • Infectious Diseases
  • Radiology

Background:

  • Xanthogranulomatous pyelonephritis (XGP) is an uncommon renal infection, accounting for 1% of cases.
  • XGP complicated by hepatic abscess is exceptionally rare, with limited literature.
  • The case involves a 54-year-old female with diabetes and recurrent UTIs presenting with flank pain and fever.

Observation:

  • A 54-year-old female with diabetes and recurrent UTIs presented with flank pain and fever.
  • CT revealed bilateral obstructive nephrolithiasis and a liver abscess.
  • Initial drainage and ureterorenoscopy were followed by CT findings suggestive of XGP with liver invasion.

Findings:

  • The patient developed Xanthogranulomatous pyelonephritis (XGP) with liver invasion.
  • A right nephrectomy was performed with an uneventful outcome.

Implications:

  • This case underscores the importance of considering XGP in patients with complicated pyelonephritis and hepatic abscess.
  • Early diagnosis and appropriate management are crucial to prevent severe complications and nephron loss.
