    Summary

Hepatitis B virus carriers can safely take isoniazid for tuberculosis prevention. Studies show isoniazid does not cause greater liver damage in carriers compared to non-carriers, making it a viable treatment option.

    Area of Science:

    • Hepatology
    • Infectious Diseases
    • Public Health

    Background:

    • Tuberculosis (TB) and Hepatitis B are prevalent in Southeast Asia.
    • Refugees from this region often carry Hepatitis B virus (HBV).
    • Isoniazid, a TB prophylactic, can be hepatotoxic.

    Purpose of the Study:

    • To investigate if isoniazid causes increased liver damage in HBV carriers.
    • To compare hepatotoxicity in HBV carriers versus non-carriers receiving isoniazid prophylaxis.

    Main Methods:

    • Cross-sectional and longitudinal studies were conducted.
    • The study population comprised Southeast Asian refugees in Philadelphia.
    • Serum alanine aminotransferase (ALT) levels were used to measure liver damage.

    Main Results:

    • Isoniazid did not cause greater liver damage in HBV carriers.
    • Both carriers and non-carriers experienced transient ALT elevations during early therapy.
    • Baseline ALT levels were higher in HBV carriers than non-carriers.

    Conclusions:

    • Chronic Hepatitis B virus infection is not a contraindication for isoniazid prophylaxis.
    • Isoniazid can be used safely for tuberculosis prevention in HBV carriers.
    • Monitoring ALT levels is advisable during the initial phase of isoniazid therapy for all patients.
