    Background:

    • Anoxic-ischemic brain damage can lead to severe neurological impairment.
    • Electrographic status epilepticus (ESE) is a critical condition characterized by continuous seizure activity on electroencephalography (EEG).
    • ESE can occur without obvious motor seizures, posing diagnostic challenges.

    Purpose of the Study:

    • To describe a clinical sign predictive of electrographic status epilepticus in comatose patients.
    • To evaluate the association between specific eye movements and ESE following anoxic-ischemic brain injury.
    • To differentiate ESE from other EEG abnormalities in this patient population.

    Main Methods:

    • Case series of 10 comatose patients with anoxic-ischemic brain damage.
    • Clinical observation of eye movements, specifically brisk, small-amplitude, vertical movements.
    • Correlation of clinical eye findings with electroencephalographic (EEG) recordings.
    • Inclusion of 2 additional patients with similar eye movements but without ESE.

    Main Results:

    • Ten patients with anoxic-ischemic brain damage presented with ESE and no significant motor seizures.
    • A specific clinical sign—brisk, small-amplitude, mainly vertical eye movements—was predictive of ESE.
    • The outcome for these patients was unfavorable, with a poor response to anticonvulsant drugs.
    • Similar eye movements were observed in 2 patients with abnormal EEG spikes but without ESE.

    Conclusions:

    • Brisk, small-amplitude, vertical eye movements are a potential clinical predictor of electrographic status epilepticus in comatose patients with anoxic-ischemic brain injury.
    • ESE in this context is associated with poor outcomes and limited response to treatment.
    • While suggestive, these eye movements can also be present in other conditions, necessitating EEG confirmation.
