Return-to-work programs for stroke survivors often neglect mood and fatigue management. Including these elements in rehabilitation may significantly improve return-to-work rates.

Background:

  • Return-to-work is crucial for stroke survivors' quality of life and social integration.
  • Failure to return-to-work leads to negative psychological and social outcomes.
  • Current rehabilitation programs often overlook mood and fatigue management, despite their impact on work return.

Purpose of the Study:

  • To systematically review randomized controlled trials (RCTs) of stroke return-to-work programs.
  • To examine the inclusion and impact of mood and fatigue management components.
  • To report on screening tools used for mood and fatigue in these programs.

Main Methods:

  • Searched 7 electronic databases for RCTs published in English up to January 4, 2023.
  • Included 5 RCTs with a total of 626 participants.
  • Conducted a narrative synthesis of intervention design and outcomes.

Main Results:

  • Only 3 of the 5 studies incorporated mood and fatigue management.
  • Studies including mood/fatigue management showed higher return-to-work rates in 2 out of 3 cases.
  • Programs without mood/fatigue components did not show significant return-to-work improvements.
  • Mood or fatigue screening tools were used in 3 studies.

Conclusions:

  • Rehabilitation programs for stroke survivors' return-to-work inadequately address mood and fatigue.
  • Mood and fatigue are significant predictors of return-to-work.
  • Future programs should integrate psychological support and fatigue management, along with consistent screening.
