    Lead fragments in joints rapidly release lead into the bloodstream within months. Lead poisoning from bullet fragments is most dangerous shortly after injury, with later cases linked to joint wear and skeletal resorption.

    Background:

    • Bullet fragments can cause lead poisoning.
    • The rate of lead release from implanted fragments is not well understood.

    Purpose of the Study:

    • To investigate the rate and pattern of lead release from implanted lead fragments in vivo.
    • To determine the primary risk periods for lead poisoning from bullet fragments.

    Main Methods:

    • Surgically implanted lead discs enriched in different natural isotopes into canine knee and muscle tissue.
    • Monitored lead release into blood via mass spectrometry over 3 years.

    Main Results:

    • Lead in knee joints showed rapid dissolution in synovial fluid, peaking in blood at 4-6 months, then declining due to encapsulation.
    • Lead in muscle tissue exhibited slow solubility post-implantation.
    • Risk of lead poisoning is highest within the first month after injury.

    Conclusions:

    • Acute lead poisoning risk from numerous fragments is highest within a month.
    • Chronic lead poisoning from joint-implanted fragments years later is attributed to joint surface wear, skeletal storage, and resorption.
