Background:

  • Postpartum depression (PPD) is a significant mental health concern affecting women after childbirth.
  • Exercise-based interventions are increasingly explored as non-pharmacological treatments for PPD.

Purpose of the Study:

  • To systematically review and rank exercise interventions for postpartum depression (PPD) management.
  • To provide evidence-based estimates for future research on exercise for PPD.

Main Methods:

  • A systematic literature search was conducted across major databases for randomized controlled trials (RCTs) up to May 2023.
  • Bayesian random-effects models were used for pooled effect estimation, with evidence quality assessed using the Grading of Recommendations, Assessment, Development, and Evaluations (GRADE) framework.

Main Results:

  • Twelve RCTs involving 1260 women demonstrated that exercise significantly treats depressive symptoms (SMD: -0.81).
  • Pram walking (SMD: -1.00), yoga (SMD: -0.73), and supervised mixed exercise (SMD: -0.77) were effective compared to usual care.
  • Indirect comparisons suggested pram walking may be superior, though not statistically significant; evidence quality ranged from very low to moderate.

Conclusions:

  • All evaluated physical exercises are effective as alternative or complementary treatments for women with PPD.
  • Pram walking appears to be the most effective exercise intervention for improving postpartum depression symptoms.
