    Area of Science:

    • Dermatology
    • Infectious Diseases
    • Microbiology

    Background:

    • Primary cutaneous nocardiosis is a rare skin infection.
    • It often presents with varied clinical manifestations, including cellulitis, pustules, and lymphocutaneous syndromes.
    • Nocardia species are aerobic, Gram-positive bacteria found in soil and water.

    Purpose of the Study:

    • To report on a series of primary cutaneous nocardiosis cases.
    • To describe the clinical presentation, causative organisms, and treatment outcomes.
    • To assess the prevalence and clinical behavior of this infection.

    Main Methods:

    • Retrospective review of seven patients diagnosed with primary cutaneous nocardiosis over 20 months.
    • Clinical data collection including patient history, physical examination, and treatment.
    • Microbiological identification of Nocardia species from clinical specimens.

    Main Results:

    • Six of seven patients had a history of local trauma or yard work.
    • Clinical presentations included cellulitis, pustules, pyoderma, and lymphocutaneous syndrome.
    • Nocardia brasiliensis and Nocardia asteroides were the identified species; two cases resolved spontaneously, and others responded to sulfonamides with or without trimethoprim.

    Conclusions:

    • Primary cutaneous nocardiosis may be more common than previously recognized, especially in patients with a history of trauma.
    • The disease typically presents a benign course with good response to treatment or spontaneous resolution.
    • Prompt diagnosis and appropriate management are crucial for favorable outcomes.
