Summary

Mpox infection can cause rare but severe neurologic issues like transverse myelitis, leading to acute flaccid paraplegia. Early prevention and public education are crucial to combat mpox complications.

Background:

Neurologic manifestations are common in mpox (monkeypox) virus infection.
Transverse myelitis is a rare but serious potential complication.

Observation:

A case of longitudinally extensive transverse myelitis presenting as acute flaccid paraplegia in a patient with recent mpox diagnosis is detailed.
Diagnostic work-up included serological tests, cerebrospinal fluid (CSF) analysis, and magnetic resonance imaging (MRI).

Findings:

The patient received treatment with tecovirimat, high-dose steroids, and intravenous immunoglobulin, followed by plasma exchange.
Despite aggressive treatment, minimal neurologic recovery was observed.

Implications:

This case highlights the severe neurologic sequelae that can arise from mpox infection.
Emphasizes the critical need for public health initiatives focused on mpox prevention and education to mitigate such outcomes.
