Summary

Vertebral fractures are common in acromegaly, despite normal bone mineral density. These fractures correlate with lower bone density but not trabecular bone score, indicating unique osteopathy in acromegaly patients.

Background:

  • Osteopathy in acromegaly presents a high prevalence of vertebral fractures (VF).
  • Standard osteoporosis diagnostic criteria are often unmet due to normal bone mineral density (BMD) in acromegaly patients.

Purpose of the Study:

  • To investigate the prevalence of vertebral fractures and assess bone health markers in acromegaly patients compared to hypogonadal patients.
  • To determine the influence of gonadal status and disease activity on bone health and fracture risk in acromegaly.

Main Methods:

  • Cross-sectional study involving 71 acromegaly patients and 75 hypogonadism patients.
  • Assessed bone turnover markers, dual-energy X-ray absorptiometry (DXA) for BMD, T and Z scores, and trabecular bone score (TBS).
  • Included spinal X-rays to evaluate vertebral fractures.

Main Results:

  • Vertebral fractures were significantly more frequent in acromegaly (29.6%) than in hypogonadism (9.3%).
  • Acromegaly patients exhibited higher BMD but lower TBS compared to hypogonadal subjects.
  • Lower BMD was associated with VF in acromegaly patients, independent of gonadal status or disease activity.

Conclusions:

  • Vertebral fractures are a frequent complication of acromegaly, linked to reduced BMD.
  • Acromegaly patients demonstrate distinct bone characteristics (higher BMD, lower TBS) compared to hypogonadal individuals.
  • Disease activity and hypogonadism do not significantly impact BMD, TBS, or VF prevalence in acromegaly.
