  • A 71-year-old woman presented with a nonproductive cough and dry mouth.
  • Autoantibody screening revealed elevated anti-Ro/SSA antibodies (240 U/mL) and positive antinuclear antibodies.
  • Chest CT showed bilateral lower lobe infiltrates.

Findings:

  • The clinical presentation and laboratory findings were consistent with interstitial pneumonia.
  • Elevated anti-Ro/SSA antibodies are a key indicator for Sjögren disease.
  • The infiltrative shadows on CT scan suggest early-stage lung involvement.

Implications:

  • This case highlights the importance of considering Sjögren disease in patients with unexplained interstitial pneumonia.
  • Early diagnosis and management of Sjögren disease-related ILD can potentially prevent disease progression.
  • Further investigation via fiber optic bronchoscopy with bronchoalveolar lavage is warranted for definitive diagnosis.
