Summary

Surgical treatment shows a higher success rate for impacted mandibular second molars (M2M) compared to orthodontic treatment alone. Both approaches, however, can effectively manage M2M impaction, allowing clinicians to select the best option for individual cases.

Background:

  • Mandibular second molar (M2M) impaction is a common eruption disorder requiring effective therapeutic strategies.
  • A systematic review and meta-analysis were conducted to evaluate various treatment modalities for impacted M2Ms.

Approach:

  • A PRISMA-guided search identified relevant clinical trials up to January 2023.
  • Methodological quality was assessed, and meta-analysis compared success rates of orthodontic, surgical, and surgical-orthodontic treatments.
  • Outcomes included treatment success, time to repositioning, and complication rates.

Key Points:

  • Surgical treatment demonstrated a significantly higher success rate for M2M uprighting compared to orthodontic treatment alone (OR = 4.97).
  • No significant differences in success rates were found between surgical and surgical-orthodontic treatments, or between orthodontic and surgical-orthodontic treatments.
  • Third molar removal did not show a significant correlation with M2M uprighting success.

Conclusions:

  • Both orthodontic and surgical management strategies can effectively treat impacted M2Ms.
  • Clinicians can select the most appropriate treatment based on individual patient needs and case characteristics.
